Gastroesofageální reflux a refluxní choroba jícnu v dětském věku

Refluxní choroba jícnu (RCHJ) zahrnuje onemocnění vznikající na podkladě patologického gastroesofageálního refluxu (GER). Patří do skupiny dysmotilit trávicí trubice, v našem případě jícnu. S rozvojem nových diagnostických a terapeutických postupů došlo i k renesanci zájmu o toto onemocnění. Je nyní často nazývána jako nemoc 3. tisíciletí. Svá specifika má novorozenecké a kojenecké období, kde často panuje nejednotnost v názvosloví, diagnostice i léčbě. U větších dětí a adolescentů není podstatný rozdíl patofyziologický při vzniku GER, RE (refluxní esofagitidy) a GERD (RCHJ) u větších dětí a dospělých.

GER je zpětný návrat obsahu žaludečního do jícnu. Ve své podstatě jde o jev fyziologický, může, ale také nemusí vyvolat změny sliznice jícnu. U kojenců může být jediným příznakem ublinknutí. Na druhé straně mohou vzniknout na sliznici jícnu patologické změny pod obrazem refluxní esofagitidy (RE), ale i změny mimo jícen, nejčastěji na respiračním traktu s rozvojem obrazu GERD (gastresophageal reflux disease)
