The authors present a case report of a 17-year-old boy with hypertension and a history of metacarpal bone cyst. Hypercalcemia, hypophosphatemia and high concentration of parathormon was found, so the diagnose of primary hyperparathyroidism was supposed. Scintigraphy and CT investigation revealed probable adenoma of parathyroid gland in ectopic localization in superior front mediastinum. This diagnosis was confirmed histologicaly after thoracoscopic resection of tumour. Until surgery, hypercalcemia was treated by hyperhydratation, loop diuretics, bisphosphonate and cinacalcet. After parathyreidectomy, "hungry bone syndrome" with hypocalcemia appeared, that was treated by calcium substitution and vitamin D. Further, the patient was subjectively without any problems, serum concentrations of minerals and parathormone were in normal range. Lasting hypertension was probably of combined etiology because of genetic predisposition and overweight.
